Completed forms should be submitted through your My Cardinal Connect student portal, under the Financial Aid tab. For your own security and to protect the privacy of your student account, the Financial Aid Office can no longer accept forms via email. Some forms may not be uploadable through your My Cardinal Connect portal; those forms can be submitted below. All filled forms must have a real "wet" signature; electronically signed forms will not be accepted. You will need your Cardinal ID & Password combination to log in for form submission. l
***If you or your family have experienced a significant income reduction since filing your FAFSA (i.e., loss of employment, medical expenses, separation/divorce), you should consider submitting a Special Circumstances form. Deadline to submit form, with all documentation, is prior to the first day of class for the term that you are enrolling. Please complete the Special Circumstances form and upload it to the link above.***
The portable document format (PDF) requires Adobe Reader®. Adobe Reader is available free of charge from Adobe. You may download it here. Once you have downloaded the latest version of Adobe Reader you can access a PDF document.
