2023 - 2024 Forms
Form Submission:
Completed forms should be submitted through your My Cardinal Connect student portal, under the Financial Aid tab. For your own security and to protect the privacy of your student account, the Financial Aid Office can no longer accept forms via email. Some forms may not be uploadable through your My Cardinal Connect portal; those forms can be submitted below. All filled forms must have a real "wet" signature; electronically signed forms will not be accepted. You will need your Cardinal ID & Password combination to log in for form submission.

Special Circumstances
If you or your family are experiencing extreme changes to your financial situation (i.e., loss of employment, medical expenses, separation/divorce), we may be able to adjust your 2023-2024 FAFSA to reflect your current situation. Please complete the Special Circumstances Form and upload it to the link above.
Dependency Change
If the Department of Education has determined you are a dependent student and you:
- did not report information about your parent(s) on your FAFSA
- and/or indicated that you may have a circumstance that requires you to follow up with your financial aid administrator
Please read the requirements of the Dependency Change Request form, and complete if needed; completed forms and any supporting documents can be uploaded to the submission link above. The Financial Aid Office cannot determine your eligibility for financial aid until more information is provided. Please be advised that the submission of this form does not guarantee any changes to your financial aid awards.
Aid Cancellation
In the event that TVCC has already processed your aid for a semester, it is the responsibility of the student to withdraw from all course registration and cancel any requests for aid or aid awarded.
(This form should not be completed unless directed to do so by the TVCC Financial Aid Office.)
