In addition to meeting the minimum preliminary requirements, all applicants must also meet ANY TWO out of the following criteria. Please check the two that apply to you (Documentation is required unless otherwise noted):

______.Minimum of a 3.3 GPA with at least twelve credit hours (Honors students must have a 3.0 minimum, if you have college credit, but a 3.3 GPA satisfies one of the two criteria). No documentation necessary if this is a GPA from TVCC. Any other transcripts that you want considered must be attached.

You must complete this application form and provide all supporting data by Wednesday, September 17, 2008 for the review process to be completed in a timely manner for the fall 2008 semester. This deadline will be strictly enforced.
